The Behavioral Health Resource Specialist works in partnership with the Behavioral Health Consultant to assist patients and their families to achieve maximum physical, social and emotional health. Provides resource/referral information about community resources including, but not limited to; rent and utility assistance, medical insurance, Medicare, Medicaid, SNAP benefits, medication assistance, and access to care. Assists families in accessing transportation and lodging resources. The person in this position must be able to demonstrate strong patient advocacy skills and follow patients through periods in which they may have of intense multiple needs. This position requires innovative and creative thinking, self-directed work style, high level of energy, flexibility, time-management, communication skills, and drive.
This position provides support to the OHSU Internal Medicine Clinic at Marquam Hill.
Advocates for patients and families, screens for abuse, suicide or medical neglect and makes appropriate referrals.
Provides support to patients and families referring to SW as appropriate.
Facilitates in the coordination of follow up care, health education, housing, transportation other needs as necessary.
Assists team with administrative duties and general office tasks including retrieval and return of phone calls, electronic messages, faxes, copies and completing paperwork and referral forms.
Maintains accurate and timely documentation of patient care activities in an efficient manner per departmental policies.
Complete all OHSU mandatory education requirements and activities within established timelines. Yearly competencies as identified by department.
Participates in meetings, workshops, seminars, and training sessions to maintain professional competency.
