This position is full-time and works approximately 40 hours per week, Monday through Friday, hours vary.
Position is responsible for coordinating the care of patients for a designated service throughout the continuum of care; serves as a clinical resource in area of specialty or expertise. Position supports the inpatient nephrology clinical service.
PRIMARY DUTIES AND RESPONSIBILITIES:
Coordinates the placement of new dialysis patients.
Coordinates the placement of dialysis patients when the modality changes.
Assists with the scheduling of follow-up appointments.
Assists with the scheduling of and performance of native kidney biopsy procedures.
Coordinates the transition of care of dialysis patients to Rehab and Long Term Care Facilities.
Assists hospital staff with questions related to dialysis patient care.
Assists outside care providers with questions regarding the care of dialysis patient admitted to the hospital.
Assist the renal team in procuring biopsy slides from an outside insitution.
Provides patient education.
Assists social workers and case managers with complex discharges involving dialysis patients.
Attends weekly ESRD meetings.
Graduate of an accredited nursing program with 2 years of nursing experience.
Current Missouri RN licensure in accordance with the Compact Act.
Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements).
Working knowledge of techniques and methodologies used in developing, coordinating, and managing clinical research projects with working knowledge of database management, spreadsheet, and statistical analysis software.
The hiring range for this position is $53,165 - $74,443 annually.
