Family Medicine at Gabriel Park is a community clinic located in SW Portland. We serve all types of patients from birth to end of life. Our services include maternity care (including deliveries), pediatric care, sports medicine, acupuncture, minor procedures, and health maintenance with preventive care. We are a high volume clinic that sees up to 200 patients and takes 200-400 phone calls daily. We have 30 providers plus 50 staff members. The OHSU Clinical registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The Clinical RN's care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The Resource RN will provide quality care management and triage nursing care to patients consistent with OHSU Family Medicine at Gabriel Park\u2019s care philosophy: patient-centeredness, advanced access, providing a robust medical home for patients, team-based care and empanelment. The RN will set a positive example of nursing for the Family Medicine residents, staff and visiting students. The RN serves in a clinical leadership role to the Gabriel Park team. The RN ensures that the clinic functions smoothly and efficiently while keeping the patients\u2019 well-being paramount. The individual represents the patients, providers, staff, the clinic and OHSU. They will conduct themselves in a friendly, courteous and professional manner when involved with any activity associated with OHSU. The presentation of medical care is an important component of the quality of medical care.
Responsible for the delivery of appropriate, timely and beneficial care for patients which promotes quality and cost-effective health care outcomes working within established standards for case management practice, and Advance Access principles. Provides standard nursing care for patients throughout the age continuum. Responsible for active management of patients. Maintains compliance with applicable laws such as client confidentiality, Americans with Disabilities Act, Workers\u2019 Compensation, abuse reporting, principles of consent, and advanced medical directives. Advocates for the client/family at the service-delivery level and at the policy-making level fostering the client/family\u2019s decision-making, independence, and growth and development. Integrates factors related to quality, safety, efficiency, and cost effectiveness in planning, delivering, monitoring and evaluating client care promoting the most effective and efficient use of human and financial resources. Functions as the liaison between referral providers and GP providers. Participates in clinic, department, OHSU campus, teams and committees. Assist in the implementation of nursing system that supports the mission of the American Academy of Ambulatory Care Nursing. (AAACN).
Expected abilities of the Resource RN :
Ability to perform duties using theory-based nursing practice model, evidenced-based practices, and shared governance in a multidisciplinary team model. Ability to participate in a multidisciplinary clinical team. Knowledge of clinical quality improvement models and process. Knowledge of scope of patient care services provided by other clinical service lines and the referral process. Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care. Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., JCAHO. Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions. Knowledge of clinic practices and ability to act as a resource for clinic personnel and assist the providers as necessary. Proficiency in the use of appropriate EMR, computer based software and office equipment. Ability to maintain quality safety and infection control standards. Resource RN must possess the ability to apply the operational principles of the GPFM care model. Knowledge and ability to apply primary care best practices. Ability to mentor and orient new staff members.
Provides accurate and timely documentation of care of patients in the patient record - EMR. Assure proper transfer of patient care and coverage of duties when leaving the clinic. Initiate appropriate interactions with patient\u2019s, providers and other staff regarding any relevant patient care or operational issues.
