Recruiting for 2 Linkage Navigators; for sites: BGMC, DMH and MFSH emergency departments. These individuals will be expected to work on-site in the emergency department and to assist patients with positive test results realted to HIV or HCV and to be linked to care. The Linkage Navigators will collaborate with local health departments, DIS workers, and local/regional health information exchange to ensure timely linkage to care.
Data collection and entry Responsibilities: 1.To review HIV and HCV test results and inform patients with positive test results by phone 2.To educate patients on HIV and HCV diagnosis. 3.To schedule specialty clinic appointments and follow-up with patients and specialty clinics 4.To provide support (e.g., translation) services as needed 5.To collect and enter patient information into the EMR dashboard 6.To recommend changes based on trends, challenges and successes
